Applying a lower hit rate than the historical attack still produces roughly twenty-five successes from sixty or more torpedoes. Some weapons would be directed against cruisers, while Duilio, Littorio, Vittorio Veneto, and other exposed battleships could receive repeated hits. Twenty-five hits across a limited target set make multi-hit casualties plausible even under conservative assumptions. The Pugliese underwater-protection system could mitigate individual explosions on some battleships, but not every ship carried it and the system was not designed for this concentration of damage.

Twenty-five torpedo hits against Italian protection
A conservative projection of twenty-five hits from sixty or more torpedoes creates repeated-hit casualties beyond what Italian underwater protection was designed to absorb.
